Appointment System

Our Appointment System

We use a "triage" system to assess your needs - this means that a member of our clinical team will assess your needs before allocating an appointment. Use this link to submit an online request for a medical problem  https://accurx.nhs.uk/patient-initiated/B86007/flow/medical-request

When completing the online forms please provide as much relevant information as possible so that the clinician can accurately assess your need. You can still contact us by telephone but we will deal with your request in the same way.

You will then be contacted with the most appropriate appointment which can be with a number of different clinicians including:

  • GP , Nurse, or Health Care Assistant 
  • Physiotherapist
  • Clinical Pharmacist
  • Social Prescriber 
  • Community Pharmacist

 

We understand change can be challenging, but we are committed to providing you with a safe and efficient service. Our new strategy will help meet your needs effectively.

 

Please also tell us:

  • If there’s a specific doctor, nurse or other health professional you would prefer 
  • If you would prefer to consult with the doctor or nurse by phone, face-to-face, by video call or by text or email
  • If you need an interpreter
  • If you have any other access or communication needs

Home Visits

Home Visits

Requests for home visits should ideally be made before 10:30am.

You should contact the surgery directly, either by phone or through the online portal, and explaining the reason for the request

When making a request for a visit our receptionist will ask you for a brief detail of your name, address and telephone number and a description of the problem, this will enable the GP to assess the urgency of the request.

Be prepared to provide information about the patient's symptoms, medical history, and any relevant details that will help the GP assess the need for a visi

Home visits are for people who are housebound or are too ill to leave the house. A patient is considered housebound when they are unable to leave their home due to physical or psychological illness, or disability, and require significant assistance to do so, even with minimal support like a wheelchair or escort. Essentially, a housebound person cannot access healthcare services at a clinic or GP surgery.

We request that all other patients attend the surgery rather than request a home visit because of the extra time home visiting takes.

On average four to five patients can be seen in surgery in the time it takes to do a single house call. In addition, the care that can be offered due to the lack of adequate lighting, examination facilities and equipment means that you may not receive as good a service as the doctor may be able to offer if you came to the surgery.

Please note that the doctor may telephone you rather than visit you if this is medically appropriate. Ultimately it is the doctors right to decide whether or not a visit is appropriate for a particular set of circumstances.

Extended Access Appointments

We have  an Extended Access Service at Oakwood Lane Medical Practice on a Saturday 9-3 , and telephone appointments throughout the week providing  the following apointments :

  • Face to Face with a GP, Nurse or Healthcare Assistant (Saturday only)
  • Telephone appointments with Clinical pharmacist
  • Initial telephone appointments with  Physiotherapist
  • Telephone appointments with a Mental Health Support Worker via the Healthy Minds service 

 

Please note, this is not a walk-in service – you still need  an appointment booked  via the surgery. You may be offered these appointments as part of our online triage service.  Extended Access appointments are for routine general practice issues and not for urgent care.  Please contact the surgery online or by telphone for more information .
