Community Matron
John Lynch - Community Matron based at Park Edge Practice and the Seacroft PCN. John works with the practice to Identify patients at risk of unplanned admission to hospital, the team enable each patient to have a personalised care plan based on their needs and provide end of life care for long term conditions.
Community Midwives
Our midwife and is part of a team who offers ante-natal and post-natal care.
Community Nurses
We have District nurses who work alongside our GP’ and Nurses. They are based at the Seacroft Clinic.
Health Visitors
We have health visitors who work alongside or GP’s and Nurses. They are also based at the Seacroft Clinic.
