Our practice list is OPEN to new patients

If you live in our practice area (please use the map to check) and want to register we need certain information from you and each member of your family so we can register you and get your previous medical records. 

You can register online https://gp-registration.nhs.uk/B86007 . You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Paper forms are still available if you need one  - Contact our reception team here 0113 2733733 or visit reception.

All new patients will be offered a New Patient Health Check.

Non-urgent advice: Temporary Registrations

A temporary registration with a GP allows you to access local medical services for limited time while you are away from your usual GP surgery. This is typically for up to 3 months, allowing you to remain registered with your permanent GP, but also be seen by a temporary GP for immediate or ongoing care during your stay. 

You'll need to fill out a temporary registration form (GMS3) at the local GP surgery. 

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

 

Non-urgent advice: Accessing someone else’s information

As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.

To requests proxy access:

  • collect a proxy access form from reception

Non-urgent advice: Linked profiles in your NHS account

Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.

The NHS website has information about using linked profiles to access services for someone else.
